Birth & Death Records
The Erie County Health Department has certificates of all births and deaths that have occurred in Erie County from December 1908 through present. Birth/Death Certificates are $24.00 per certified copy payable by cash, money order, credit card and certified bank check. To obtain your certificate, you can either come into the office Monday through Friday, from 8:00 AM to 5:00 PM (excluding holidays) at 420 Superior Street, Sandusky, Ohio 44870, charge by phone, or send in a request to us.

Request for Birth Certificate must include the following information:
- Birth Name
- Date of Birth
- Current Address
- Current phone number
Request for Death Certificate must include the following information:
- Name of deceased
- Date of Death
- Current Address
- Current phone number
$24.00 per copy payable by money order to the Erie County Health Department.

Genealogy
Uncertified Certificates are no longer available: Effective July 1, 2003, we will no longer issue uncertified copies of vital records such as births, deaths, fetal deaths, or stillbirths. Only certified copies of these documents will be available for issuance. The change is reflected in the repeal of Section 3705.23(D) of the Ohio Revised Code. Birth and Death Certificates are $24.00 per certified copy payable by money order and list the following:
- Either the birth name or name of deceased
- Date of birth or date of death
- Current mailing address
- Current telephone number
