Birth & Death Records

The Erie County Health Department has certificates of all births and deaths that have occurred in Erie County from December 1908 through present. Birth/Death Certificates are $24.00 per certified copy payable by cash, money order, credit card and certified bank check. To obtain your certificate, you can either come into the office Monday through Friday, from 8:00 AM to 5:00 PM (excluding holidays) at 420 Superior Street, Sandusky, Ohio 44870, charge by phone, or send in a request to us.

Genealogy

Uncertified Certificates are no longer available: Effective July 1, 2003, we will no longer issue uncertified copies of vital records such as births, deaths, fetal deaths, or stillbirths. Only certified copies of these documents will be available for issuance. The change is reflected in the repeal of Section 3705.23(D) of the Ohio Revised Code. Birth and Death Certificates are $24.00 per certified copy payable by money order and list the following:
